I have compiled a list of websites that I believe will be helpful to you on your journey. This is by no means an exhaustive list, but rather a tour of the types of resources that are out there.

The Conversation Project

Its “starter kit” and other tools are dedicated to helping you begin the conversation with friends and families about their EOL wishes.

Go Wish

A website utilizing videos of people thinking about their EOL preferences to walk you through the steps of choosing a surrogate decision maker, determining your own preferences, communicating these preferences. It functions as an online coach, and motivates you to get started on the process.

Death Café

Gatherings of people, often strangers, at a variety of coffee shops and yogurt bars in your area, with a downloadable guide to the conversation.

Death Over Dinner

Organized dinners for whoever is interested, with prompts and materials provided to encourage and facilitate deep communication and conversation.

National POLST Paradigm

A website that provides state-specific POLST forms

Five Wishes

The Five Wishes form functions as an advance directive in most states, but uses more ordinary language and focuses more on personal and spiritual priorities.

Advance Directive Forms for your state

My Directives

This website is similar to Prepare for your Care in helping you think about your preferences, but goes further by helping you to create an actual Advance Directive which can be emailed to your doctor and family.
